1235 15 Sqn 1k, 1inj Rusayo, between Beni and Goma, DRC Whilst flying on a UN mission as UN821, a bullet penetrated the front cockpit window, injuring the Commander and fatally injuring the Flight Engineer. The crew landed safely at Goma Airport.

1227 OK Beni area, DRC Flying alone as UN820, the aircraft came under fire. The pilot in command performed evasive actions, avoiding the ground fire but clipping three of the rotors on a tree. All three rotors have damaged tips and need to be replaced, but the aircraft is otherwise undamaged.

1209/UN827 UN 15 OK Goma airport, Democratic Republic of the Congo. The helicopter was taxing on the ground when the nose wheel hit the raised section of a resurfaced area and the nose collapsed onto the ground.

1213/UN820 UN OK Beni, north-eastern Democratic Republic of the Congo The helicopter was flying the MONUSCO Force Commander on a routine visit to the Bunia area of operations when several shots from a small calibre weapon were fired at it from unidentified gunmen on the ground. One of the projectiles penetrated an internal fuel tank, causing a small fuel leak. The aircraft commander took evasive action, flew out of the danger zone and diverted to Beni Mavivi Airport.

1200 TFDC 3 OK ?25nm NE of Sutherland Experienced a total loss of fluid from the main hydraulic system, leading to the aircraft losing all secondary hydraulic functions (normal landing gear lowering, brakes as well as the auto pilot system). This caused severe degraded controllability as well as an inability to lower the landing gear fully (Right hand main landing gear remained locked in the up position).

The Oryx executed an emergency landing on a farm, landing with the right hand landing gear on a stacked slate stone pile. aircraft was repaired on site the next day.

1249 22 Sqn 3 OK Open field in Kempton Park, near JIA. Prior to landing at the Denel facility as number two in a two ship formation about 14h00 local time, experienced a total tail rotor failure while transitioning into the hover at 40 feet AGL. The helicopter spun and hit the ground hard in an open field near the airport, causing the aircraft roll over.
